What to Bring to the Hospital

  • Your ID and the patient’s ID (if available)
  • Health card and/or insurance information
  • List of medications, allergies, and medical conditions
  • Copy of Power of Attorney or advance directives (if applicable)
  • Notebook or phone for notes
  • Contact list of family members, doctors, and employer

 Helpful Reminders

  • Stay calm and take notes.
  • Don’t hesitate to ask for plain-language explanations.
  • Request updates regularly and politely.

and, perhaps the most important thing to remember...

Look after yourself — eat, rest, and get  support. 

Find that relative, neighbour or long-time trusted friend who will just be there with you.

 That one calm person will make an amazing difference.
